Crohn's & Colitis UK want to find out what parents and younger people think of their NHS IBD service in Scotland - just the key points.

We will use the trends identified to support any need for further improvements in standards of care for all younger people with these conditions in Scotland.

This is a short, anonymous survey that will only take a few minutes to complete.

If you have already been asked these questions at one of our recent Family Days there is no need to continue with this questionnaire.
