My IBD Care helps people with Crohn's or Colitis to access support and resources, set medication and appointment reminders, and self report their condition giving a greater sense of empowerment. A cloud-based dashboard gives healthcare professionals, whose NHS trust has signed up for the app, to understand how well, between appointments, someone with Crohn's or Colitis is and how effective different care pathways are. My IBD Care was developed by Ampersand Health in partnership with Crohn’s & Colitis UK and the gastroenterology teams at King's College Hospital and Barts Health.
The HSJ Partnership Awards honours and recognises the works of the most effective partnerships between the private and third sectors and the NHS. Annually, the companies that are most committed to improving patients’ lives, healthcare services and delivery are celebrated and applauded.
Empowering people living with Crohn's or Colitis in their own care has benefits to everyone involved. Care can be delivered in a more responsive and personalised fashion, enhancing both the experience and effectiveness of treatment; and reducing the number of hospital visits and overall healthcare utilisation.
The app is used by thousands of people living with Crohn's at a growing number of participating hospitals to improve communication, knowledge and self-management. It was developed by Ampersand Health and trialed at King’s College Hospital during a surveillance study which ran for six months and included 54 people living with Crohn’s or Colitis. The study results were positive, 85% of people in the trial stated they would prefer to use the app as the primary method of clinical contact for routine checkups.
The app is free for patients and can be downloaded from the iOS and Android stores.
