By now you might have heard your doctors and nurses say the word ‘transition’. They’ve probably already explained that it means moving from the children’s to the adult IBD team. This may be in the same hospital you go to now, or a different one.
Maybe lots of changes are happening all at once – exams, college, university, jobs, friendships, relationships, sex, going out, money…and much more.
You're not alone. We have lots of information to help you get through the transition from children's to adult care and help you tackle other aspects of your life whilst living with Crohn's and Colitis.
